Working Papers

 

1. Parents MUST pick up the applications for son or daughter.

2. Certificate of age—MUST be shown.

3. Application MUST be completed by both the physician and the employer.

4. Working Papers can be obtained during the following hours

 

                          Monday thru Friday    7:45 - 11:00 am

                                                                  12:45—3:15 pm

             Please bring identification with you.
